LASIK commonly involves two technology steps

A technology comparison should therefore identify both the flap-creation method and the excimer treatment plan. A brand name in one step does not describe the entire procedure.

Femtosecond laser: creating the flap

A femtosecond laser delivers very short pulses that separate corneal tissue at a planned depth. It is used to create the flap in all-laser LASIK. This changes the method of flap creation; it does not remove the need for the excimer laser that reshapes the cornea.

Ask for the femtosecond device model and the surgeon’s plan for flap thickness and diameter. “Bladeless” is a useful category label, but it is not a promise of a particular result or freedom from flap-related risks.

Wavefront-guided and wavefront-optimized are not synonyms

Wavefront-guided treatment uses measurements of how light travels through the eye to build a treatment plan that can account for certain higher-order optical aberrations. Wavefront-optimized treatment generally starts from the glasses prescription and uses an algorithm designed to preserve corneal shape characteristics.

The FDA notes that wavefront-guided treatment measures subtler distortions than conventional treatment, but patient selection and surgeon competence remain central. Ask which method is proposed and why it is expected to matter for your measured eye—not whether the center owns a machine with “wave” in its name.

Topography-guided treatment maps the corneal surface

Topography-guided planning uses a detailed map of corneal shape. The map and refraction contribute to a customized ablation plan. Philadelphia Eye Associates, for example, identifies Contoura Vision topography-guided treatment with an Alcon WaveLight suite on its official site.

Topography-guided does not mean universally preferable. The approved indication, measurement quality, prescription and surgeon’s judgment all matter. Ask whether the treatment is on-label for your case and what alternative plan would be used if measurements are inconsistent.

SMILE uses a different corneal approach

SMILE creates a small piece of corneal tissue, called a lenticule, within the cornea and removes it through a small incision. It does not create a LASIK-style flap and does not use an excimer laser for the refractive correction. Local centers that advertise SMILE should identify the exact platform and the prescriptions they treat.

PRK is another flap-free corneal procedure, but it removes the surface epithelium before excimer treatment and follows a different healing pattern. “Flap-free” therefore does not identify one procedure.

Five technology questions worth asking

A useful technology discussion should end with a specific treatment plan. Bring these questions to the surgeon, not only to a sales counselor.

  • What are the manufacturer and model of every laser used?
  • Is the proposed use within the device’s FDA-approved indication for my prescription?
  • Is the plan wavefront-guided, wavefront-optimized or topography-guided?
  • What measurement would make you change or postpone the plan?
  • May I read the device patient booklet before consenting?
