General contact and listing-correction forms deliver messages to a private BookMyLASIK business inbox. Consultation lead collection for listed centers is not active.
Information not requested
Do not submit medical histories, prescriptions, diagnoses, medications, insurance information, eye conditions, dates of birth or other sensitive health information. BookMyLASIK is not a medical provider or patient portal.
Contact messages
When you submit a contact or listing-correction form, BookMyLASIK collects the name, reply email, selected topic, message, and any profile URL you choose to include. The information is used to review the request and reply manually. FormSubmit processes the submission for email delivery. Its public documentation states that form submissions may be retained for up to 30 days. Review the FormSubmit privacy information for details.
Traffic and technical information
BookMyLASIK uses Google Analytics 4 with measurement ID G-R4BC7QX9MR to understand visits and use of the directory. Google Analytics may receive page URLs, approximate location derived from an IP address, device and browser information, referral information, and interaction events. Names, email addresses and contact-form answers are not added to analytics events. Google Tag Manager and call tracking are not configured.
Future consultation inquiries
No center is currently authorized to receive BookMyLASIK leads. If consultation requests are introduced, the form will identify the recipient center or centers before submission, use a separate secure system and display approved consent language. Lead details will not be sent to analytics.
Cookies and analytics choices
Google Analytics may use cookies or similar identifiers. Browser privacy settings, content blockers and Google’s available controls may limit this collection. BookMyLASIK does not use analytics data for medical decisions and does not intentionally send names, email addresses, phone numbers or form answers to Google Analytics. See Google’s privacy policy.
Your choices and contact
You may use the contact form to request access, correction or deletion of a message you submitted. The formal business address, final retention procedure and jurisdiction-specific rights process will be added when finalized. The business inbox address is intentionally not displayed on the website.
